In de antistofrespons tegen Coxiella burnetii ontstaan zowel fase 1 als fase 2 antistoffen. Bij verdenking op chronische Q-koorts worden IgG fase 1 en IgG fase 2 antistoffen bepaald. De diagnose chronische Q-koorts wordt gesteld op een verhoogde titer van IgG fase 1 antistoffen: ≥ 1:1024, in afwezigheid van acute Q-koorts. In dat geval wordt er aanvullend ook een Coxiella burnetii PCR verricht op het serum om de aanwezigheid van Coxiella burnetii DNA aan te tonen of uit te sluiten.
Aan- dan wel afwezigheid van Coxiella burnetii DNA in het serum wordt gebruikt bij de indeling mogelijke, waarschijnlijke of bewezen chronische Q-koorts.
